State Mandated Screenings and Exams

Pennsylvania School Law establishes the required screenings and examinations for all students within our public school system.  Below you will find a listing of the screenings completed by the School Nurse at each grade.  Annually you will receive notification of your child's BMI and any abnormal screenings. Also listed are the private exams and the grade level at which they are required.

Vaccination Requirements

Students attending Pennsylvania schools must be immunized according to the following schedule, in order to attend school. 

Students entering Kindergarten are required to have:

  • 4 doses of tetanus (one after age 4)

  • 4 doses of diphtheria

  • 4 doses of polio (one after age 4)

  • 2 doses of measles

  • 2 doses of mumps

  • 1 dose of rubella (German measles)

  • 3 doses of hepatitis B

  • 2 doses of varicella vaccine (chickenpox) or history of disease

Students entering 7th grade are required to have:

  • 1 dose tetanus, diphtheria and pertussis (Tdap) within the last 5 years

  • 1 dose meningococcal (MCV)

Students entering 12th grade are required to have:

  • 2nd dose meningococcal (MCV) after 16 years of age

These immunizations will be verified for any new students enrolling in West Perry at any grade.
