The Pennsylvania Interscholastic Athletic Association (PIAA) has issued guidelines for athletic physicals, which West Perry School District follows. West Perry athletes must complete the PIAA's Comprehensive Initial Pre-Participation Physical Evaluation form known as the CIPPE form. The guidelines are as follows:
Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, at any PIAA member school in any school year, the student is required to
SUBSEQUENT SPORT(S) IN THE SAME SCHOOL YEAR: Following completion of a CIPPE, the same student seeking to participate in Practices, Inter-School Practices, Scrimmages, and/or Contests in subsequent sport(s) in the same school year, must complete Section 7 (Parent Re-certification) of this form and must turn in that Section to the Athletic Trainer of his or her school. The Athletic Trainer will then determine whether Section 8 need be completed.
ImPACT Testing: Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, at any PIAA member school in any school year, the student is also required to take the ImPACT Test, complete and return the ImPACT Consent Form if they fall under one of these categories.
