New Student Enrollment
Welcome to the West Perry School District!
We’re happy to welcome you to the Mustang family.
We use an online student registration program, which allows parents/guardians to input student information when or where it’s convenient to them. Parents/Guardians wishing to enroll or re-enroll a student must follow the instructions below.
Complete and submit the online enrollment form (link above) using PowerSchool Enrollment
- If you have used this system before, please log in using your existing account. If this is your first time using this system, you will need to create an account (please keep this information for possible future use).
- If you are unable to complete the online enrollment form all at once, you can save your work and log back in at a later time to continue where you stopped. An online enrollment form must be completed for each child you are enrolling. After the first child is added, you can click the link to ‘Add’ an additional child.
- In order for your child’s registration to be processed, you must submit the online enrollment form.
- Children registering for 1st grade must turn six years old BEFORE September 1st of the school year of enrollment.
- If you do not have access to internet or need assistance with completing the online enrollment form, please contact Mrs. Dodson (info below) to make an appointment at the District Office.
Submit all required documentation as part of the online enrollment process. If necessary, these documents can also be emailed to Mrs. Dodson.
- Proof of your child’s age: original birth certificate, notarized copy of birth certificate, a valid passport, notarized/certified record showing the date of birth, notarized statement from a parent or relative indicating the date of birth, prior school record indicating the date of birth
- Immunizations required by law: the child’s immunization record, a written statement from a former school district or medical office, verbal assurances from the former school that the required immunizations have been completed with records to follow
- Proof of residency showing you live in our district (two required): deed, lease, sales agreement, notarized statement from landlord, mortgage information, driver’s license or DOT identification card, utility bill, current credit card statement, permit to carry card, vehicle registration
- Photo ID of parent/guardian: This is used to verify the person’s identity that is completing the registration. It is also required to notarize documents such as sworn statement attesting if the student has been or is suspended or expelled for offenses involving drugs, alcohol, weapons, infliction of injury, or violence on school property.
- Special Education documents (if applicable)
- Court Orders / Custody Agreements (if applicable)
- Unofficial transcript (grades 10-12) or final report card (grade 9) from former school
Additional Information –
- Who can enroll a child? Parents or court-appointed guardians living in the WPSD may register a child. Exceptions apply to homeless youth (according to the McKinney-Vento Act), migrant, and foster students. A student living with someone other than a parent will be registered in accordance with guidelines set by PA Public School Code section 24 P.S. 13-1302. Residents other than the parent or court-appointed guardian will be asked to complete and sign an affidavit stating that they and the student live in WPSD on a full-time basis.
- Who needs to submit an Affidavit of Multiple Occupancy? If you are registering a child with WPSD and you are residing with a District property owner, an Affidavit of Multiple Occupancy should be completed by the owner or lessee of the property. The parent/guardian of the child should complete an Application for Multiple Occupancy. If this situation applies to you, please reach out to Mrs. Dodson to get these forms.
- Do I need to submit the required documentation if my child is eligible for services under the McKinney-Vento Act? The documentation requirements listed above may not pertain if your child is eligible for services under the McKinney-Vento Act. PLEASE NOTE: While every effort is made to enroll students as soon as possible, please be advised that it may take up to five school days after we receive necessary documents to complete the registration process.
