New Student Registration

IMPORTANT ANNOUNCEMENT: West Perry School District will be closed for Winter Break from December 24 - January 2. During this time, new student enrollments will not be processed. You can still submit the online enrollment form, however, it will be reviewed beginning January 3, 2023. 

To register your child with West Perry School District, you must complete the online enrollment form and contact the District Office. If you are registering a student for grades 9-12, you may be required to make an appointment with the High School counselors prior to the child's start date.

A parent or guardian must pre-register their child by completing the online enrollment form.

    • The online enrollment form is completed using PowerSchool Enrollment. If this is your first time using PowerSchool Enrollment, you will need to create an account.
    • An online enrollment form must be completed for each child. After the first child is added, you can click the link to 'Add' an additional child.
    • 2022-2023 enrollment form 
    • You should upload the required documents (see below)  while completing the online registration. If necessary, these documents can also be emailed or dropped off at the District Office. 
    • If you are unable to complete the online registration all at once, you can save your work and log back in to continue where you stopped.
    • If you do not have internet access or a computer, please contact the District Office to reserve a time to complete this process. Please have the required documents with you for this appointment.

**Children registering for Kindergarten must turn five (5) years old before September 1.
**Children registering for 1st grade must turn six (6) years old before September 1.

Required Documents for Enrollment

  • Proof of your child's age: Any one of the following constitutes acceptable documentation: original birth certificate, notarized copy of birth certificate, a valid passport, notarized/certified record showing the date of birth, notarized statement from a parent or relative indicating the date of birth, prior school record indicating the date of birth.
  • Immunizations required by law: Acceptable documentation includes: the child's immunization record, a written statement from a former school district or medical office, verbal assurances from the former school that the required immunizations have been completed with records to follow.
  • Proof of residency (two required): Acceptable documentation includes: a deed, lease, sales agreement, notarized statement from landlord, mortgage information, driver's license or  DOT identification card, utility bill, current credit card bill, permit to carry card, vehicle registration. If residing with a district property owner, the property owner must sign a notarized "multiple occupancy form."
  • Photo Identification of parent/guardian: This is used to verify the person's identity that is completing the registration. It is also required in order to notarize documents such as sworn statement attesting if the student has been or is suspended or expelled for offenses involving drugs, alcohol, weapons, infliction of injury, or violence on school property.

Requested Documents for Enrollment

  • Other documents: These documents are necessary in order to appropriately schedule students for classes. Report cards/transcripts (required for students in 9-12), all Special Education documents (IEP, ER, RR, NOREP), Service Agreements (504), Gifted documents, attendance records, any other records relevant to your child's education.

PLEASE NOTE: Until the required documentation is received, we cannot finalize a registration or enroll a student in the District. 

Who can register a child

Parents or court-appointed guardians living in the West Perry School District may register a student. (Note: Exceptions apply to homeless youth according to the McKinney-Vento Act, migrant and foster students.) A student living with someone other than a parent will be registered in accordance with guidelines set by PA Public School Code section 24 P.S. 13-1302.

Residents, other than the parent or court-appointed guardian, will be asked to sign an affidavit stating that they and the student live in this District on a full-time basis.

If you are registering your child with West Perry School District and you are residing with a district property owner, an Affidavit of Multiple Occupancy should be completed by the owner or lessee of the property. The parent/guardian of the child enrolling here should complete an Application for Multiple Occupancy.  If this situation applies to you, please call to get these forms.
