 Required Exams
Student in grades 7, 3 and Kindergarten must have a completed dental exam on file with the school.
 Students in grades 11, 6 and Kindergarten must have a completed physical exam on file with the school.
 
Vaccine Requirements
All students enrolling in West Perry in grades K through 12 must have the state required immunizations (see left side bar)  Students in 7th grade and above are also required to have a tetanus (Tdap) and meningitis booster.  
  
 Feeling Ill?
The school nurses/healthroom aides work on the assumption that you have made the decision your child is well enough to attend school.  Please consider keeping your child at home if he or she: has a fever of 100 degrees or higher (without being medicated), has been vomiting, has a severe sore throat, or other symptoms that would prevent him or her from participating in class.  Changes to Immunization Requirements
for 2017-18 School Year
 1. Proof of vaccination must be provided within the first 5 days of school (August 30th)or risk exclusion for all grade levels.
2.  Seniors must provide proof of at least one dose of meningitis vaccine (Menactra, MCV) after their 16th birthday.

School physicals are due for all students in Kindergarten, 6th and 11th grades.  If you have a private physical exam form completed, please turn it in to your school nurse ASAP.   Failure to turn in a mandatory physical may result in loss of privileges such as parking permits, Prom tickets, club participation, etc.
 
School dentals are due for all students in Kindergarten, 3rd and 7th grade students.  If you have a private dental exam form completed, please turn it in to your school nurse ASAP.     Failure to turn in a mandatory dentals may result in loss of privileges such as club participation, etc.  
 
 SPRING SPORTS PHYSICALS
There are no more scheduled school physicals for athletics for the spring season.  All physicals must be completed privately at this time.
 
CHIP Applications are available
