Sports Physicals

The Pennsylvania Interscholastic Athletic Association (PIAA) has issued guidelines for athletic physicals, which West Perry School District follows. West Perry athletes must complete the PIAA's Comprehensive Initial Pre-Participation Physical Evaluation form known as the CIPPE form. The guidelines are as follows:

INITIAL EVALUATION: Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, at any PIAA member school in any school year, the student is required to

  1. complete a Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE); and
  2. have the appropriate person(s) complete the first five Sections of the CIPPE Form.
    1. Upon completion of Sections 1-5 by the parent/guardian, and Section 6 by an Authorized Medical Examiner (AME), those Sections must be turned in to the Principal, or the Principal's designee, of the student's school for retention by the school. The CIPPE may not be performed earlier than June 1st and shall be effective, regardless of when performed during a school year, until the next May 31st.

     

SUBSEQUENT SPORT(S) IN THE SAME SCHOOL YEAR: Following completion of a CIPPE, the same student seeking to participate in Practices, Inter-School Practices, Scrimmages, and/or Contests in subsequent sport(s) in the same school year, must complete Section 7 (Parent Re-certification) of this form and must turn in that Section to the Athletic Trainer of his or her school. The Athletic Trainer will then determine whether Section 8 need be completed.


ImPACT Testing: Prior to any student participating in Practices, Inter-School Practices, Scrimmages, and/or Contests, at any PIAA member school in any school year, the student is also required to take the ImPACT Test, complete and return the ImPACT Consent Form if they fall under one of these categories.

  1. Any student in grades 9-12 who have never taken the ImPACT test while at West Perry High School.
  2. Any 9th grade athletes who have not taken the ImPACT Test in the Current year. (i.e. If a 9th grade athlete participates in a fall sport, they will be tested that season. If that same student participates in a Winter or Spring sport, they will NOT need to take the test again.)
